The contact form of a WordPress website doesn’t work.
Php mail doesn’t work, the sender adress has to be from the domain.

I have a wordpress site for a client and it has a contact form 7 form that you can see here: http://bind.digitalcoaching.nl/offerte-aanvragen

I need the fields to display in the right place. some of the fields are not positioned right.

Also i need the email that is send after submitting to change.

I want the same layout from the form to be also in the email that is send.
so there needs to be created a full email layout for this form!
(because the standard email is very hard to make up what options are selected)

We require someone to add a besoke .php contact form to our existing website (wordpress based). The form must have a feature for users to upload documents.

We will supply the design & fields in word and jpeg format.

The form must also work with our Google conversion code.
