Clients need to:

Download and save a copy of the form and complete it offline on their own pc.

Attached is the pdf, there are a few checkboxes and the others are for text.

I need this asap. Let me know if you have done this before, and how fast this can be done.

I have a psd that I need turned into a html site using only css and javascript. It has to be finished in one day and this has to be held in strict confidence, you will need to sign a non disclosure agreement, and can not show this in your portfolio or represent it as your work anywhere. I have a tight budget for this, but will have lots more work if you do a good job. It is a fairly complex design with a slideshow and video on another page.
Will pay when project is complete.

I have a 35 ppt slide show that I need turned into a video that should be around 2:00 – 2:30 minutes.

I will do voice over after so based on info on screen the timing might be a little longer or shorter. Sort of read at a normal pace just to get the timing.

I would like some nice transitions between slides and where appropriate a bit of animation – just to make it a bit more interesting.
