I need a form created for my website. I attached a PSD file of the form.
All validations in red, should be placed as seen on the PSD file if the user types in incorrect info.

We need a joomla specialist for our website who can create joomla 1.6 extensions.

The hired person will be required to create extensions which will enable users to apporve a form or entry.
For example:
>User 1 creates the form
>User 2 will then be notified about it.
>User 2 has the choice of approving or disapproving the form

I had a project done here at scriptlance but unfortunately, even though I paid the provider, he has gone awol and won’t respond to my requests to complete the project.

He has completed 99% of the job but I need the form he made to send me an email when a customer submits it..currently the form is setup to allow a customer to input their information and then it sends them to paypal to make payment. that needs to continue but in addition I need it to also send me an email with the contents of the form.

This is coded in php and I’m providing access to my hosting account so you need to have some feedback here that goes back at least a year.

I can’t imagine this job will take more then 10 minutes.

I want to create a review form like the one I’ve attached. The form needs to be created where I can send it out from my email, someone can fill out the form within their email when they “reply” and then send the form filled out with their information.

We require a database to be built to capture a form page on our website. we will require the form to be put into our admin section so that we can see details of anyone that has filled it in and in addtion the form will require to be emailed as well

there is currently only 4 fields, name, email, subject and comments. there is also a “capture” on form for submission .this is not required to be picked up.

Also after the form we also need to make the sure thank you page at referral_thanks.html is also working and that the PDF attached to the form that is supposed to populate automatically does and is emailed to the person completing the form and to us.

the form on the site has a few problems I need fixed…

1. it seems to have stopped e-mail the form data….
2. IE is OK BUT if you use chrome or Foxfire the click-able area of the forms is off alignment…
3. after hitting submit it pulls up the thankyou page BUT it goes away very fast and goes back to the form…. it should stay on the thank you page….

We created a form. The form stores it’s data in the same database as our SMF Forum. Your job is to allow forum users to fill in that form once. You must connect the form results to their forum ID.

You must also add an EDIT button on the form to allow forum users to EDIT their form data and store it again. (and overwrite earlier data)

When it works, then give the form same colours as forum.

remove business category
change county network to “which state would you like to block”

1) non register members will fill in a form on the indivdiual that integrated on to the profile user page.

2) the non member will then submit the form

3) A Thank You email will be sent to the sender and they are require to click on something (link) in the email to acknowledge the email.

In the past, I’ve done forms in FrontPage (which often stop working) and Coffee Cup form builder (which only seems to work reliably in IE). Since I’m not a programmer, I’ve never been able to create forms… So I’m hoping I can use the form from this project to create other forms myself in the future.

The form to be created for this project needs to look and work like the one shown above. It will only have one change – on page 2, it will have a ‘check’ option to say there is no co-borrower to avoid having to answer co-borrower questions.

The form would be placed in a webpage created from the client’s existing template. I assume you will be able to copy the form from the pages on the site shown, so hopefully, only minor script changes will be needed. The site is hosted on a GoDaddy Deluxe plan (Linux), in case that matters.

When we fill and submit the form, the email never arrives with the info.

Also need to make sure there is validation ( If some fields are not filled, the form should prompt user to fill the fields before form submits – Only 6 fields to validate ( Name, Email Address, Phone, From, To, Date )

This is a reposted project from yesterday.
The webpage has now been created and I am looking for some further CSS/Javascript for the search form to display and function properly.

Currently the form elements are standard ones but I would like them to look like the ones in the picture. Once completed the form must also be cross browser compatible.

I would like only a class to be applied to the form elements as I will be applying some php data to display data at a later date. The forms must be designed with this in mind as some of the data will be dependent on one another.
