I need a dynamic menu select based on AJAX(Jquery), PHP and Mysql. The required functionality is like this : (see the image attached)

1.- User select a main category of products on the first menu-select.

2.- AJAX automatically returns the list of subcategories in the second menu-select.

I have a shopping cart form that needs the customer to select ONE from two lists who they want to make a donation to (Charities A or Charities B).

Currently both dropdown lists of Charity options are shown by default. What I would like is to have two radio buttons/links that say “Select Charities A OR Select Charities B” shown by default. Then, when the customer selects A or B, the corresponding list of Charities is then shown beneath, but ONLY the selected list.

If the customer selects “A”, then “B” should be set to 0 and vice-versa.

The form html can be hardcoded, and this work should take no more than 10 minutes.

This is for online seat builder. See instructions below:

1. Visitor chooses a style “Classic” or “Pro” (I know steps in ordering instructions attached file are a little different but I see a better flow in terms of programming so I rearranged them here).
2. Based on the previous style selection, visitor will have either 10 seats models available to select from (Classic series) or 5 models (Pro Series)

By default the page will display the current year stats. The affiliates signup date is stored in the database.

When a new user signs up and enters their zip or postal code, I would like a list of nearby cities to appear. I’d also like those cities to appear on a map. I’d like the user to be able to select cities by clicking on a checkbox next to the city in the list, or by clicking on the city on the map.

When the user returns to the page, I’d like them to be able to de-select cities in the same manner.

Ideally, I’d like the map to be able to segment down to the zip code level, to allow users to select individual zip codes – but this may not be possible. I will be willing to consider higher quotes that offer this feature, so please specify if you feel you can do this.

I require a person to add Music Instruments/Equipment from one website to another. You will be given an account to log onto and will need to:

1. Select the correct ‘Category’ (drop down menu)

We need some changes to a products return database.

I need a programmer to create me a dynamic way to add many product attributes. A simple select products and move to “page” would be nice. Page should be created in admin (maybe by adding new category and having an option for a product with product attributes?)

I have a crystal with thickness 1 mm. I have diameter sizes crystals from 15 mm to 30 mm, in increments of .1 mm (15.1, 15.2, etc). Now you can see this is 151 different sizes and model numbers for the same crystal. I want the customer to be able to select these sizes (product attributes) through a drop-down menu. Once selected and added to cart, the corresponding model would be selected.

For example, Model Number C1.0/15.1 is 15.1 mm size crystal with thickness 1 mm.

Hello,i have an microsoft access form- it is a complicated form- because it uses a listbox and has a second listbox invisible – that helps the form perform its functions.

you enter a service no and hit tab or enter
you select your name and hit tab or enter
you select a part and hit enter and the part fills the listbox

If you click the button view/edit records in my ms access form you will see how records fill into database.
I Do not need the tables to work the same- actually I am not happy with way this database is set up-
BUT I DO need this to work that exact same way as MS ACCESS FORM DOES.

When the user returns to same page he/she will be able to view selection previously made and able to edit selection + add further reports so a secure login, pass reminder function, captcha is required also. Again updating db if selection has been edited or further reports added.

Options to add in bespoke info into fields should the options provided aren’t suitable.

On a seprate page we want to be able to view the reported faults, by type, make, model, and possible other paramters plus using a graph to show this data.
