Urgent–need a solution fast!

The above link references what I’m trying to accomplish. On the sample page you’ll see a button click the POST PROGRAM METRICS.
Project Requirements:
1.) In the table: I want to make the name of each program and the button in the Input Form column of each row to launch a container form as demonstrated with the example POST PROGRAM METRICS (modal:true with tabs oriented to the left, etc.).
There could be 1 program, or there could be 100 listed, so I need a dynamic solution.
2.) Once the button is clicked, I want a PHP file to request a query for any data that may exist for this particular program (via program_id) and populate the form.
3.) Once the user makes changes to the form, when they hit the SUBMIT button in the form, their action will update the database and close the container window. The parent page should display the updated changes.
4.) Finally, there is a IE bug (IE 6 and 7) that causes issues with the displaying of the container. I’d need a fix for this as well.
Also, I don’t need a full solution–I simply need an example. Using my sample page. I simply need an example that demonstrates how to do this. In other words, you don’t need to query the entire form–use a few example fields.
