I have a survey in TYPO3 Questionnaire running on a server and want to convert it to Limesurvey version.

I. An Overview
Bankers working with business clients need to have a good understanding of how those businesses operate. One of the key methods of understanding a business is to understanding how business manages their cash. To get this understanding the banker has to ask the business client a series of questions about how they get paid by their customers, how they manage and secure their money once they have received it and how they pay their bills. The banker’s underlying goal of asking these questions is to identify potential cash flow problems that are reducing the business client’s efficiency and effectiveness.

III. How It Works
There is a link on my website will take the visitor (banker) to an introduction page for the Cash Flow Questionnaire. Once on the introduction page the visitor/banker has two options:
a. Use their unique access code to sign on to their account
b. Register as a new account

If they register as a new account, they will complete a registration page that asks their name, bank/company name, business title, city and state and email address. They must select a payment plan (a free 1 month trial or 12 months annual subscription for $12). Next they must select a payment plan (Pay Pal, Visa or AmEx)

Once they have access to the Cash Flow Questionnaire, there is a cover page that gives a brief overview of the questionnaire process and lists the benefits to the business client for participating in the interview.

The Cash Flow Questionnaire consists of 25 to 30 basic questions. Each question will have 3 to 5 potential answers. Beside each answer is a small box. It is a 16 page online questionnaire.

I would like a professional looking version of the questionnaire either created in word or pages or indesign before turning into a PDF. I will need to make edits for some text tweaks later so it needs to be editable.

The questionnaire itself, when online, has text boxes that appear when the person answers yes. These need to be available in the document version as boxes for the person answering the questionnaire to complete.

There will also be an additional cover page which I already have in word to go at the front.

Each of the “pages” on online questionnaire needs to be contained within its own “page(s)” in the PDF version. So start a new page for each online page.

We need to automate a questionnaire process currently used for our customers. This project will accomplish this automation by storing the questionnaire information in an easy to access format while tracking and reporting on progress and history with each questionnaire submission.

The attached document is the basis for this form. In addition to each of the form fields, most will have an explanation below the question. There will also be some introduction text to display above the form. This text will be provided prior to completion of the project.

When someone visits this page, they must first enter their email address – this will determine if the form is new or one already in progress. No password is required for this step. If the email address matches a form already in progress, the system should open this form so the visitor can continue where he left off. If it is a new email address, a new form should be created tied to this new user name.

Each time the visitor moves to a new field or attempts to submit the form, the form data should save to the database so they can exit at any time without losing any data.

ALL fields are required. A visitor cannot complete the form 100% without some text present in all fields. Validation should be used to let the visitor know of any blank fields if they attempt to submit an incomplete form.

Notifications:

- Administrator should get a notification via email each time a new questionnaire is started.
- Administrator should get a notification via email with the full form submission text once completed 100%
- Visitor should get a notification upon 100% completion with a message defined in the admin area.

Admin panel:

- Administrator settings – update admin email address(to be used as the login as well) and password. There will only be 1 administrator for this script.
- Administrator can log in and see a list of active form submissions listed by property name and email address along with % of completion. The list should be sortable by property name and date of last activity.
- Administrator should also be able to view a list of completed submissions listed by property name and completion date. These completed submissions should also be stored and linked from this list. The administrator can view and edit the stored, completed submissions. The list should be sortable by property name and by completion date.
- Administrator can view a list of archived submissions. The list should be sortable by property name and by completion date. These can be moved back from the archive list to the completed list but cannot otherwise be edited in this list.
- Administrator can email completed submissions by entering a recipient email address and selecting “send”.
- Administrator can edit the customer notification email described above (under notifications).
- Administrator can move completed submissions to the “archive” list separate from the rest of the completed submissions.

Project requirements:
- The selected programmer can create this script on their own server or ours – we will provide an account with FTP and cpanel access to do so.
- Once testing is completed, the script and database will be migrated to another account. The selected programmer is responsible for making this move and testing after the move is completed. We will provide FTP and cpanel access at this time.
- Both our test account and the final account to host this project is a linux/MySQL server. phpinfo is attached.

Automatically dialing, a telephone server needs to be set up (this can be run offsite, on our dedicated server). Automatic dialing involves a telephone server would need to be set up, along with a VoIP provider. One (1) or two (2) of our workstation(s) would need a software VoIP telephone set up also. This would would be run offsite and accessed remotely like a website. Also, the workstations will require a software VoIP client along with a headset and microphone. For automatic dialing, a system similar to skype is used.

A telephone server called Asterisk (www.asterisk.org) needs to be set up (this can be done remotely by you using our dedicated server), and a VoIP provider (will need to be selected by you) to make outgoing calls.

This really is straightforward, and a clone of an existing system. Your charge is to make the design of the questionnaire entry and published results pages different, more elegant. I have tried to provide you with very detailed design criteria so that you can be as efficient as possible. I am always available for questions and clarifications and will work with you as much as you need.

The first component of the system is the login system. There are three levels of login rights.
I define my site users in three levels, each will have its own dashboard, all entry is password protected with a captcha field.
1…. Administrative. This is for my staff and I. We can control all.
2…. Firm level. These are my clients. They have the ability to create multiple questionnaires for use only with their clients. They can download all their clients for a particular questionnaire or a single clients questionnaire results in separate csv or pdf files. No firm has access to the files or clients of another firm.
3…. Client level. These are the clients of my clients…the firm level clients. They have access to the questionnaires of only their respective firm. They cannot see or access other clients or the other client questionnaires. They can only access the published questionnaires of their respective firm.

This project does not encompass setting up the full firm/client administration system.
Just a basic three level login control system. However, as I expect us to go all the way
with this project, as this is the first of our projects, a test project, you might consider building
in the hooks for a much more extensive administrative firm/client login system.

As I stated this is a database driven online questionnaire system whereby the questionnaire that displays is driven by an entry system that defines three critical areas.
1…. What is on a page

Simply define the questionnaire and then publish it. Multiple questionnaires can be defined and published. Publishing causes the system to create the web pages for the questionnaire, or as an option a fill in form pdf document. Once published it can not be changed, however, a new questionnaire definition can be created by the user from scratch, or by duplicating one of the stored questionnaire definitions and then modifying it prior to publishing.

There are four levels of questionnaires.

2…. Global questionnaires. Created at the administrative level and designated as global (box checked). These questionnaire designs are available to all at the firm level.

I need to build a questionnaire in Flash using action script 3. I have the design and animation part done and I need some help with the programming part. I have attached a simplified version of what am I trying to do.

Basically, I have 4 questions, 3 of which contain 2 options and 1 contains 3 options. There is a total of 24 combinations/answers. I would like to assign a number value to each option and than multiply that number with each option selected in order to get a unique result number. After the user has made the selections the calculations should result to a unique number e.g. 12. Than the script should go and stop to a specific frame number. For example “if total value is 12 go to and stop frame 100 (where I will be displaying a custom results page for that result number). Please refer to the attached image for more info.

I am looking for a product questionnaire script for my line of products.

The script should allow me to provide a series of questions and multiple-choice answers that are tallied, with the final tally pointing to one of my products. The concept is similar to a questionnaire found on a dating site, where the visitor’s answers are matched with a member’s profile.

I would like to be able to build a simple web page around the script. I’d prefer a php-based script with a MySQL database, however I’d be open to other options as long as they are compatible with cPanel.
