Need to create a template page for easy inputs into my wikipedia page (already created).

A) Users will need to open my Wikipedia page and enter very basic information into a the fields: Address, City, ST, Name of Gas Station, Price of Fuel for type of fuel.

B) Create a list of select boxes for the user to check if the Gas Station has Car Wash, Air, Service Stations, etc.

C) Upload a few images of maps and pictures of station

Once user is done they need to push a save button and the information they filled out is automatically added to GasPriceWiki.org WikiPedia database page.

I want a squeeze page template like these http://www.masscontrolsite.com and www.productlaunchformula.com I want them to be video squeeze pages. I want something that is easy to edit and ad keywords to as well. I would like a variety of video squeeze page templates as well if someone has that. Need 5 simple (but elegant) wedding templates as kinda like the one attached. Nothing real fancy but pretty looking. Only need home page of template as inside will be just like home except different content (no cutting in, no html, no css, etc. – only PSD design).
