I have a quiz containing 5 questions each with 2 or 3 answers that the user can select via radio buttons.

Once the user has selected the answers they would then click on the “submit” button at the bottom of the page.

I would like to have the opt-in form come up by a lightbox (or something that performs the same function) once the they click the “submit” button.

All questions should be “required”.

The biggest requirement is that depending on how they answer the final question I would like to send them to 1 of 3 separate lists or opt-in forms.

So, the opt-in that pops up will depend on how the final question is answered.

I would be a great added touch to have a active graphic following the “submit” button and before the opt-in form appears to show that the system is “thinking”. (see attachment)

This would also have to be 650px wide.

HOW TO APPLY:

The project I want created is a simple one page website template. The template should consist of a bordered page with a graphic header, a nice menu below the header, a youtube embed box with a text header above it,one opt-in box beside the youtube video, one header with a body paragraph video the youtube video, and 2 pictures with subtext beside the body paragraph .

I have attached the image I want you to use for the header. Please do not resize the image for the header as I want to be able to exchange header images for images of that specific size. Also, I have provided 5 opt in templates that you must use for the opt-in box. The 5 opt-in templates are all the same but different in color. Please do not edit the opt-in templates as well because I want to be able to interchange them for one another as well. Chose the blue opt-in header for the website. The opt-in box must be ready for aweber. I want to put in my e-mail address and for it to work. After someone opts-in I want the redirect to go to google.com as well. I want all links to redirect to google as well except the home link which will obviously redirect to the current page. I will change the links to google.com later to redirect to their respective pages.

If possible please send a link to previous work or possibly a screenshot of your proposed design. I have provided a guide of the objects the website must contain. Other than that please be creative in designing this template. Feel free to change the background, but the inside must remain white.

I also want a border around the youtube video embed box as well as a border for the pictures. Embed whatever youtube video you want, I will change it later. Use whatever two pictures you want as I will replace them later on with relevant pictures.

This website is a music oriented website for a DJ so keep that in mind
if you want to get creative with the template, background color and/or design.

As far as font for the headers is concerned, be creative, but I do want the body paragraph text to be black and in times new roman and a readable size.

I have an existing mobile formatted opt-in page that I want improved. I don’t have the skills to code and improve the design, but I do know what I want it to look like.

I have an HTML template that needs to be edited with my unique text and an opt in box from an email marketing program. There are just a few graphics.

Should be a quick and easy job for anyone with experience.

I need created a very basic opt-in form builder similar to Awebers or any other similar companies but a much more simplistic version.

The file attached (opt-in-form-example.png) is a screen shot from Awebers and is what I’m generally after.

The web opt-in form builder will generate HTML & CSS and will look something like the following:

- Header
- Optional text
- Input field
- Submit button

I would also like to include an optional border and allow users to modify the color scheme for every feature through a color chart of some sort.

There should also be a ‘Generate Code’ button that will popup with the HTML & CSS for the form so users can copy it to their clipboard and use it wherever they feel.

The last feature I require is that users can input a POST URL somewhere for the form.

I am using phplist with a the maxblogpress subscribers magnet. I just installed masblogpress onto the site. When you enter the site, the footer pop up ask for your email and name.. I enter it and press the subscribe button and should be lead to a thank you page, however… I am lead to phplist subscribe page with my name and email already entered, so I must press subscribe again… I need to fix this asap!

I need an AWeber opt-in form placed on the home/sales page of my membership website.

This would be a form that is already designed in AWeber, it should only take a few minutes to grab the code off their website. I don’t need anything custom made.

The name and e-mail address of the opt-in client will be saved in a list on AWeber. You would need to make sure this is functioning correctly.

After the client uses the form to opt-in, they would be taken to a thank you page and on this page would be a link to a specific page promised to the client from the sale page.

I have already created the thank you page and the page that is promised to the client. This would not be part of your job, but please test and make sure the tech side is functioning correctly.

I’m not sure, but if someone knows AWeber I would think this should be a pretty simple fix.

I need an email opt-in page converted to work with WordPress. The basic opt-in page is already designed in HTML. What I would like is to be able to use this opt-in page as the header for my site.

- The WordPress layout should be 1 column consisting of a header (the opt-in page), the article space, and a footer that accepts widgets.
