I need an online form I can embed on a order page on my wordpress site. This is just a simple form and doesn’t need to be fancy. I will need to be able to add/change fields, and the form must allow users to upload up to 3 – 4 images from their local computer. I need the form to be set up to send me an email when someone completes a online form (be nice to have it with the data already on the email), also a email gets sent to the user who completed the form, and the images that they attached I will need access to them either in my wordpress media gallery or a location for me to access to post on my site since I would manually post the data on my website after receiving it from the user.

You can either do this or set me up with a online form you have that will do all this but I would need you to make sure what I need is all there.

I will provide the Photoshop files, layout creative, text and links.

1. Design an eye catching form using specific elements from other online applications

A club instructor OR Competitor registers details
email address, name, club name, instructors name address, phone no. and Country(DROPDOWN SELECT)
Then they can add competitors details
Name, Age if 16 years or under, Grade, boy/girl/man/woman/Veteran etc. then they can select how many Sections from a set list
(Forms, Judo, Kumite, Light Continuous, Mixed Martial Arts,Semi Contact) then they select from a sub division.

Then they can repeat and compete in another section and add up to 6 sections.
If the want they can then add another competitor name and details to add more competitors.
The end options are Print & Post, Print and Email, Pay full amount online, Pay a small deposit online.
Email is sent to them and me confirming competitor entries

I need a online test to be created.

Step one – student will create profile where they make a username and password.

step two – they will pay for the test via paypal that will bring them to the login page once they have paid for the test.

step three – they will login to the test

step four – the test will start and will keep them on the page for a specified time (lets say 5 minutes) so they can not continue until the 5 minutes has passed. Once 5 minutes has passed they continue button will highlight allowing them to click continue to the next section. This process will duplicate itself for multiple pages.

Step five – they will come to a test that they must complete in order to continue. Once they complete the test and click continue the next page will show them the results of there test.

step 6 – they will be emailed the results and/or certificate for completing their test.

I need to have you build a test creator that manages test additions and also test question management.

Information must beable to be stored in database which admin can log in and see how many people regitered, took test and test scores and results.

Student must beable to have reset password sent to them via email in case they forgot their password.

Project must be delivered in agreed timeframe if not completed in specified time frame in project all escrow fees and project fees will be refunded.

I need this completed within 2 days, if project has not been completed in stated time frame a complete refund will be provided.
