Authorize.net Order Form

I need a custom order form created that will do some calculations. Then the total needs to be passed to authorize.net using PHP. Then a confirmation/denial page to be displayed. Authorize.net has sample code available for the processing.
Attached is a detail of what the order form should do. I would suspect the calculations can be done client side (javascript) and then a form total and description of items ordered needs to be sent to authorize.net
