You will create a simple webpage that (will be password protected directory) it will have a field where I will enter a customer ID for and click a “generate” button, that will go “grab” the data from our database for that customer, and create the “printable” page for me to easily print.

I will send you the source code from the current page that grabs the data so you can see how and where to reference the new page.

The designer I initially listed has done a sub-satisfactory job in my eyes. The general layout is correct: I want 3 columns, same sub-pages, Google Ads, etc… I just want it more professional. I would like the color-scheme to be black/white/gray, with some red as well. (see player below for colors)

I need a script created that clones the sign up form for a membership site. I am an affiliate for this site but I want to design my own landing page and not use theirs. Normally I can just copy and paste the code but their sign up script is a little more sophisticated. I need this done ASAP for a fair price. It looks like when you visit the site www.thesite.com/affid.php?=1234 it drops a 30 day cookie with the affiliate ID and then there are no hidden fields on the site that have the affiliate ID. So somehow it pulls data from the cookie. Anyways I need this done ASAP
