Chained Mysql Dropdowns

I am trying to create multiple dropdown menus using MySQL. The user selects from the first dropdown menu named STATE. After doing so, the next menu, CITY appears and lists all of the cities for the state that was just selected. The options are populated from a MySQL database; they are not hard-coded.
Please do not send portfolios or details about your company, as I’m after something specific. If you have a demo of exactly what I’m looking for, post it in the PMB.
