Ventrilo Status Generator/Widget

I am looking for a customer php ventrilo status generator for my website. This will enable customers to enter the ventrilo ip/host and port number and receive a widget displaying all channels/rooms, users in the server, and additional channel information.
You will be provided with a script from Ventrilo.com, which you will have to use.

This widget would have to contain a text link to MaxFrag for SEO. This text link must not be able to be removed.
- Having experience with the software is a plus. Though most will be able to do it regardless.
- Must contain options to customize the appearance (example #2 and #3)
- Must contain ready-made skins for various games and default skin (example #1)
