I’m looking for a very basic PHP/MYSQL customer loyalty script.

I want users to be able to:
Login to their account (Accounts will be added manually in MYSQL, no public registration)
Add a new customer (First Name, Last Name, Telephone Number, Customer Number and Point Balance).
Find a customer by Customer Number
Display customer’s Point Balance
Add and Remove points from the customer’s point balance

As pages go, it would be:
1) Login page
2) User home page with search box and ‘Add Customer’ button
3) Customer page with First Name, Last Name, Telephone Number, Customer Number and Point Balance
4) Add Customer page with fields for First Name, Last Name, Telephone Number, Customer Number and Point Balance

Customers are ‘owned’ by the user that added them, shouldn’t be accessible to other users.

It doesn’t have to be pretty, just functional.
