I have a survey in TYPO3 Questionnaire running on a server and want to convert it to Limesurvey version.

I have an existing survey in Word format, I want to find somebody who can setup the survey in LimeSurvey. So you would be responsible to make it look and function exactly the same except that it would in an online format. There are roughly 60 questions ranging from standard multiple choice to more advanced ranking type of questions.

I have attached the survey in the PMB.

I want a new look and feel template, I’ve already got a site and just need the look and feel copied

I would like somebody that can also built new question types.

We are looking for a someone who will customize one of free templates.
1. we need 1 quetion to be shown at a time;
2. we need to change template.css to add background and change the questions look&feel;
3.we need to add transitions between the questions with jquery ui.
