Need Help To Get Form Working In WordPress

I have converted an HTML site to wordpress. It includes a form, which has some javascript to make it work correctly. This no longer works on the wordpress site.

It might be the paths to the javascript files, or something similar.
I need some help to figure out what is wrong.
