It works fine, sending the inserted data via email –

Currently I only need the following.

1. Make sure the uploaded files are properly uploaded and sent as a zip attachment on the same email.
2. This plugin supports multiple files being uploaded so please test that this works well
3. Change the settings so all type of files are uploaded (except .exe). (Also limit the maximum file size to 24MB)

I’ll fix the styling after you’re done so no need to worry about that.

That’s all I need – simple project for someone with not too much to do. Only requirement is to deliver today.

No need for long cover letters for these simple tasks.

Hi, I need a paypal buy form created that does not leave the page youre on. So the user enters their paypal username & password and it confirms their shipping address and places the order. all without leaving the current page youre on or have it refresh. Please only bid if you know how to do this with paypal. Do not want to spend the time working with a programmer that does not know how to do it.

An edit is required of the attached script form script which functions somewhat, but is missing many components.

The purpose of the form is to be able to start entering data in any ANY of the 12 fields and have the data in ALL the fields match the chosen selection.

I.E. if a visitors enter an “Airport”, then the other fields update with corresponding cities, countries, continents, airport codes, airlines, etc with records that match the airport selected.

After each entry is made, a running count must be posted beside each field (i.e. 20 cites, 4 countries, 2 continents, etc.)

If there is ONLY 1 entry left in the ANY field it much be displayed rather than showing the option. (This works on a couple entries)

A user can then determine if they wish to narrow the searches on a second field entry, which are based on the original “airport” they’ve entered PLUS the new field.. and so on.

Form fields need to be in AJAX show as you type format (yahoo search assistant style) but all must available in a javascript dropdown “onfocus” if nothing is typed. Only when a person type should the AJAX narrow the search.

Once they complete field entry and desire a search, the data needs to be displayed in sortable and searchable columns. However, the original form must be displayed along with the information they inputted so they can edit if need to.

A full write-up with examples is included in the zip file along with the current script (minus smarty templates), database tables, etc.

I am looking for someone to program a PHP/AJAX/JS lead form for me. I need it pretty specific and be able to output into a CSV file and also have the choice to email the output real time (so I want to be able to have the choice for it to output the fields into a CSV file or change it so I can have it output into an email and email it to an email address of my choice with the filled out fields shown).

What I need is the user to enter a zip code – then it would take them (preferably via AJAX so there’s no screen refresh) to another part of the form where it displays the CITY and STATE for the zip code they entered – and then it will show contact information fields (name, address, phone, email, “type of project you’re interested in”, etc.) and then it will say Thanks for your submission – A local contractor in (whatever city and state for the zip code they entered in) will be calling you within 24-48 hours, etc… You need to have or find a database to use that will have all the zip codes match the city and state (I don’t have this)

I have a form, all fields are prepopulated so validation for empty fields is not working. I need you to correct the form validation to only accept valid values before sending.

The area in red is the section of my site that I want the action to take place in. I do not want the entire page to reload, as I will have a video playing. If the page reloads the video would start all the way over. I only want the section in red to reload. The rest of the page needs to be unaffected by that reload as the text is displayed:

The form needs to maintain it’s current functionality. The form submits it’s information to get response.

On my site I have a get response form that is asking for the name and email. I want to have a form that only displays the email field initally, once someone enters their email I want that field to disappear and the name field to appear…

Once the email is entered I would like that info to be sent to the autoresponder in the background. I would like the name field to appear without the entire page having to reload or re-freash…

Once they have entered their name I want that info to be submitted to the autoresponder as well. Finally I want to be able to use their name though out the page that I am redirecting them to once they have filled out the two fields…

So I’m guessing that the name needs to be stored in a cookie, so that it can be access on the next page. However you can code it however you like so long as the end result is the same…

I need a custom order form created that will do some calculations. Then the total needs to be passed to authorize.net using PHP. Then a confirmation/denial page to be displayed. Authorize.net has sample code available for the processing.

Attached is a detail of what the order form should do. I would suspect the calculations can be done client side (javascript) and then a form total and description of items ordered needs to be sent to authorize.net

I need a contact form script. A basic form javascript that I can paste into my webpage to receive emails from clients. I would prefer to have a form that has Captcha to preven spam.

So we need:

1. Form code so I can install myself
2. I will need instructions
3. I have to be able to just paste the code into my own design
4. I have to be able to receive data submitted via email.
5. Must have Captcha feature.

It can be ajax, but would prefer a simple php code.
