My website has a signup form that was created by “borrowing” code from another website. The form actually pulls from that sites resources. I did not realize this until now.

I have two word format forms that I have created and I need them both to be converted into fillable pdf forms using a basic pdf reader so that my clients can complete them onscreen and send them back to me without losing any of the formatting or having to print them out and fill in manually.

I have 2 forms that I need done that are 1 page each. If this works out OK I have lots more forms to be converted.

I would like new forms created from PDF documents that I have. I need the PDFs that I currently already have re-created with the same information on them, but with editable form fields so that I can fill in directly with my Ipad or PC. I don’t want to have to print them out and write by hand anymore…Being able to type in the fields and fill it in with any basic adobe pdf reader would be perfect.

I have approximately 5 forms that I need done with a total of about 10 pages or so.

I have all the parameters and a basic design for a form I need to be created. I only know basic PHP and need this completed for me so that I can start Capturing Leads.
