Form Fix

Hello,

We have a form at http://www.elikaassociates.com/referral_broker that is two parts however once selecting submit it does not goto the second part referral_form_2.html. Also after the form we also need to make the sure thank you page at referral_thanks.html is also working and that the PDF attached to the form that is supposed to populate automatically does and is emailed to the person completing the form and to us.

Looking at fixing this asap.

Fix Form Fields In Landing Pg

I would like some help with a landing page that I am putting together. I need someone to align all the form fields so that they fit correctly under the part of the graphic that says “Get Your Free Bottle Now”. Fields should fit and have a yellow color. You will also need to make sure that it looks good (alignment and color) in Firefox, Internet Explorer and Safari. You can use http://browsershots.org/ to check the other browsers.

Fix Existing Form

Description:
Fix existing service request form at www.macymaid.com (http://www.macymaid.com/request.html). The customer fills out an online service request form, submits data, which then the data is captured in our MySQL database at www.macymaid.com. The data can be view by a Macy Maid Administrator. An email is sent to both the person requesting service and to the Macy Maid Administrator email address.

The Process and What You Are to Deliver:

1 ) Customer Enters Data on a form and submits.
2 ) Their Data is captured on MySQL Database.
a) Macy Maid can view the Customer Data (Service Request) at a secure login webpage in chronological order.
3 ) Both customer and Macy Maid receive an email about the service request made by the Customer.

Fix Form

Hi there,
We have a form done in PHP and MySQL. It is a 5 step ordering form which goes page to page in 5 Steps.

Problem is when a user makes there selections on step 1, step 2, step 3, step 4, etc and if they click the back button on the browser, all the selected and entered data is gone. The user has to start from Step 1 again.

We need it fixed so when the back button is clicked anytime in any browser, the users selection and entries will remain on the previous steps and NOT be wiped out.
