I have a PDF file that i need to be made into a OFFLINE fillable pdf file.

Clients need to:

Download and save a copy of the form and complete it offline on their own pc.

Attached is the pdf, there are a few checkboxes and the others are for text.

I need this asap. Let me know if you have done this before, and how fast this can be done.

I have two, two page scanned pdf files. I need these to be recreated as form fillable. I want to have buttons at the top of the page to print, save and/or email the forms. The only format I have them in is as a scanned image. See attached files.

This is the functionality I’m looking for:
There is a template with various sections. The sections will include: Demographics, HPI, Social/Family Hx, PMH, Medications, Exam, Data, Impression and Recommendations. I click on a section, such as ‘demographics’ or ‘Exam’ and it opens up the template for that section. There are fillable text fields that can be filled via keyboard or speech-to-text. There are radio buttons for yes/no. There are some drop-down fields. I enter data, and then go to the next section. When I’m done, I save the template with an unique file name. The sections in which no data were entered don’t show up in the final file or printout. I can send it to a printer or faxserver which I can choose from a menu of destinations, depending on my location that day. Ideally I can tell it to print a bunch of saved files at once. (typically 5-10 at a time).

Anl example of my current paper form is attached. I’ve added red boxes and text to indicate where fillable fields and radio buttons should be used. The detailed arrangements are flexible, but basically all the information and sections that are shown on the attached the two page form must be in the fillable form.

ABSOLUTE REQUIREMENTS:
1. format can be modified occasionally
2. can be printed or sent to a fax.
3. will contain fillable fields (text boxes) and yes/no checkoff sections (radio buttons)
4. It can run via 3G at times, though sometimes wifi will be available
5. When printed/saved, the document will look like a normal letter or note. It won’t show all the boxes around the fillable fields. Empty fillable fields and their titles won’t be printed. For instance, if I check ‘noncontributory’ and nothing is entered in the box under ‘family history’, then the empty box won’t be saved/printed.

DESIRED REQUIREMENTS (negotiable)
1. certain radio buttons, when checked, will open a fillable field. For instance, look at the sample template. When I check ‘abnormal’ for a section on the musculoskeletal exam, I need to have an explanation. That could go somewhere under the array of radiobuttons, most of which will be checked as ‘normal’.
2. The file name should be generated by three of the fields in the form. That is, note that there are fields for ‘firstname’, ‘lastname’ ‘date’. When it’s time to save the file, it should automatically generate a file name of ‘lastname’_'firstname’_yearmonthday. I should be able to edit that if needed, but I want to streamline and take human error out of the file naming process if possible.
3. The ‘Medications’ fillable field can take input from a database (I’m looking into that).
4. It has the capacity to either a. open multiple files simultaneously so that data can be put in in parallel, or b. after saving, the files can be re-openned and edited/completed.

please let me know in your proposal whether or not you can meet the absolute and/or desired requirements as detailed above.

4. There will be several sections of the form. Each section will have a radiobutton that can ‘turn off’ that section so that it is blank or not saved in the final file and the printed copy. For instance, if I’m not documenting the medication list, then I don’t want a section that says ‘medications’ in the final file. So maybe there can be a radio button that can be clicked to exclude it. Or maybe it will be automatically not included if I don’t enter any text? your choice. let’s talk about it.

I have 105-110 mostly pdf forms. Most of them are non-fillable. I need that clients on our website be able to fill them in on-line. 1/3 of these forms need some additional work to be done, like removing some text, getting the form from the 3-rd party web page, converting to PDF.

We need a fillable PDF form for our website. The form needs to compute total cubic feet. Sample of what we are looking for is attached.

We will need to take a couple of columns out of the form. All we need blank are Number of Items and Cubic Feet. We need the form to compute the Cubic feet by # of each item and give a total at the end of the form. We need for the customer to be able to fill out and email the completed form to us.

Basically I have designed a website, and it uses community builder for their memberships, but the main thing is that I want to add FORMS to the website,
The main thing is that the forms must be:
1)AUTO FILLABLE with the person/user’s name etc.. so that the user doesnt have to always type in their name, d.o.b (their info pulled from the Community Builder fields)etc when they are logged in to send that form…
2)When the form is sent to the respective person in charge..the person will log into joomla to view the form and APPROVE it,…the approve could be a print for example..

3) OPTIONAL:- The person who approves the form, the form (in dbase form) could be sent to another central place where the records in that form can update a database…

I need to have the attched pdf file converted to be a fillable form pdf file. asap.

This is a relatively simple project to convert the fields of a one-page medical intake form (currently a scanned PDF) into a fillable form. Please see the attached form to see the form fields.

Please note that the final product will be a form that a user can download and fill in using Adobe Acrobat Reader and then allows them to select a ‘Print this Form’ button so they can take this form to the office.

I have a 10 page form build in Microsoft Word 2007 (Docx) that I need converted into a fillable PDF form so people can fill in the blanks and send it back into us. This should be a simple project for someone with a professional version of Adobe Acrobat. Our goal is to create something that’s very easy for our customers to print and fill out or complete directly on the form and email it back to us.

I would also be interested in turning this into a form our clients can fill out online and have it record their answers in a database. We currently have a Joomla! site and don’t do much with the MySQL or contact records section.

You can bid to to either the PDF only or BOTH the PDF and online form solution (we will eventually need both but our most urgent need is the PDF.)
