I am in the finance sector and want to save time with populating forms that contain the same fields (i.e. name, address, telephone number etc…).

Typically, I(or my referral sources) fill out a client application(which I created in Microsoft Excel and converted into PDF form) by hand and then use the information contained within this client application to populate other required forms. This is time consuming and what I would like is a piece of software that I (or my referral sources) can execute and type in the required information and then print all the required forms with the all the fields correctly populated.

All I have are the forms in PDF format. So, “I guess” they would have to be all scanned with OCR and then create the software so that the appropriate data fields can be populated in all the forms. There are some pages than do not need any populated data, however, I would like to have these pages in the software because it would be easier to just input the required information into the software and then press a key and all the pages would then be printed.

The whole goal is to print all the required pages in one shot with the appropriate data fields populated. There will be some data fields that will still need to be populated, however, that can be written in by hand by the individual.

I would also like to have all the code and a list of the software used to create this executable file, so that in the future I can call on you or if you are not available to give it to another coder to make modifications.

There are 17 pages in total, with 9 pages that require no population of data (they just get printed out with the modified forms)

Please let me know what you would charge to do create this and the estimated time frame to complete. If you have a better way to do this, let me know your thoughts or recommendations. PC based application is preferred or if not that difficult have it be able to run on an apple machine also.

This project is to create a program for the Android Marketplace. It will have five fields on the GUI (only for Android phones and tablets) and the fields are for example, “Date” “AM or PM” “City” “State” “telephone number of user” (a constant).

Database should be built in to give User choices to put into fields. The purpose is for the user to locate indivduals in other cities who meet certain date and time requirements.

I am using CMS Made Simple, specifically a module for completing orders. The order form requires fields like Address, City, State, Country, etc. I want to remove these required fields and make only firstname, Lastname and email required.

I need the attached PDF turned into a PHP form that emails to the user. Basically, you are taking the contents of the form, and turning it into a page that the user can enter the info and it will email to an address that i designate to you.

The layout of the form fields should look similar to the PDF. I will send you the template file where you can insert the form fields. All fields are required so there should be validation. The validation should be easy to edit.

The email output should be formatted exactly like the form itself (
I don’t need the header and footer, but the agreement # and company property name should be on one line, then contact, phone #, and fax # should be on another line, etc etc)

i’d like to have a button created in flash. This button will do 2 things:
1. if you click on it for the first time, it will pop up a box that will have a form in it. this form will ask for Fist name, Last name and email address. This form will submit those information as cookies.

2. if you click on it for the second time and you haven’t clear you cookies, this button will be a submit button. it will have the stored information as hidden fields, that would pre-populate the fields, so if somebody clicks on it, it’ll give them new box, that would say. Thank you. We’ve got it. Please check your email for download instructions.

So to make it simple: if there are no cookies stored, it’ll be as a form that stores the data into cookies, if there are cookies, it will pre-populate the hidden fields (first name, last name and email) and submits the form after clicking on it.

in our prestashop site (ecart.me) we have a problem in adding a new address form. When we press on Add New Address, the form does not open fully and the first two form fields ( first name and last name) remain hidden.

Lightbox is built into our theme, however it like all shortcodes don’t work in custom fields. We tutorial that says it’s possible, but were unable to implement it ourselves.

I have a MySQL database of products and I need a simple application that will let me populate two fields in the table based on who the product provider is.

It doesn’t need to have a pretty backend, just something simple where I enter the provider number and then two text fields so I can populate the “manufacturer” and “google_taxonomy” fields.

Once I click “insert” I would like those fields to be populated for all products in the database with the specified provider number.

The changes required for this website are divided in 3 tasks.

Task 1 Description:
The website has a press section that displays a list of press cuttings. This list needs dividing in two sub-categories: UK Press and International Press. All existing press cuttings must be classified in either of the two new sub-categories. When the site’s Admin adds new press cuttings, he must be prompted to select between UK Press and International Press (the two new categories).

Task 2 Description:
One of the PHP-based forms that appears on the website currently posts data to an e-mail address. It is necessary for this form to also post to a CSV file or to a Google Docs file (prefferably Google Docs, as it would update instantly).

Task 3 Description:
It will be necessary to make amendments to 3 of the PHP forms. Mainly: add a drop-down menu with a “specify which” field, make some fields mandatory (must be filled in), and correct one error in one of the forms (one of the fields is not posting the data inserted).

That’s it! They are all relatively simple changes but we would like them to be made by a DRUPAL / PHP specialist to ensure everything is perfectly programmed.

I have a small html web site on a windows server and I need a form mail installed on my contact page.
