The problem is I don’t want everlasting forms, so I want only the next employment details to appear when the value of years is less than <3. If the figure is 3> years then that part of the form doesn’t need to be shown. I would like the developer to look at my forms before quoting and see if they can create the following. If the employment details is sub <3 then I want the user to be taken to a further stage and then complete on wards until he has completed over >3 years. This must also work the same as all my other forms and be dropped into the database in the same way. Please ask further questions if you need to.

I need a simple application (web or stand alone, doesn’t matter) which captures all the input fields for an employment application (name, address, previous employment history etc.).

The idea is to convert a paper employment application into a web form/app that prospective employees can fill out and have their information captured in a database. We then would use this information to automatically populate all employment forms (tax forms, confidentiality agreements, policy and procedures acceptance form etc.) with the persons name, Social security number, address and any information required by each particular form, so if the person is hired this would not have to be manually input.

We have all the forms in digital form (.doc, .pdf) so it would be a task of loading the forms in the system/app, mapping the respective fields required for each form and being able to print out the forms by selecting a persons name from the database of applicants.

I can send the forms for review and analysis if anyone needs them or contact me if you need additional clarification.

I have student registration form that captures data in about 8-9 pages. The scripts has 2 pages that capture education details and employment details. These 2 are repeats, i.e., the student can enter as many as education details he wants.
