Looking for a simple form to database (either online or off), basically a mailing list however there are quite a few fields including image uploading.
The form would contain the following:
8 x text fields
3 x drop downs
1 x text area
1 x set radio buttons
1 x set checkboxes
4 x photo uploads
entries should go in to table, photos into thumbs to open up full size, with the ability to easily mail individuals, groups of groups or everyone.
Im happy to pay for your time to find or modify and existing script, just want it to work.
Please quote for both styling and not styling the form as I can do that if it’s a job.
Lastly the page will be in html5
