I need a custom graphic form designed. The image is pre-designed (22 slices). I need someone to create the table with appropriate background images and submit fields.

I am in the middle of developing a website. There will be many forms and application pages that will contain all sorts of form fields (i.e. text field, drop down boxes, checkbox, radio button, etc.)

I am really trying to go for a custom look and feel for the site and thus want to develop these form fields to look branded and sleek. I have seen such branded form fields on other sites and believe that there is someway to bypass the standard web browser’s rendering of these fields.

My graphic design team has already developed the vector images for these fields, I just need someone with programming no how to insert these graphic and override default browser field rendering.

I need form validation on some html pages – these can be converted to PHP if necessary..

Validation will include fields like US Zip code (5 digits) and username which will be letters/numbers only (no underscore or spaces, etc)

and some drop-down validation as well.. not very many fields, should just take a few minutes for someone that knows what they’re doing – would prefer some nice css/ajax looking stuff.

I have a page on the website for a custom spicekit (http://www.spicekits.net/customkit.htm). I need help creating a form or something that integrates with paypal where a user can choose 10 or 20 spices from a list of spices. They can choose the same spice more than once. For example, they can choose cumin 10 times if they would like. They must first choose if they want the 10 spicekit or the 20 spicekit and the price will change accordingly. Then they choose which spices they want in their kit. I am not a programmer so I have no idea how to do this and how to integrate it with paypal. I am leaving this solely up to the programmer to figure out. Please do not ask me if I want the information emailed to me etc. I DO NOT KNOW. You have to help me figure this out.
