I am looking for someone to customize the login and registration forms on the following page so they look more like my site theme:

Modifications Required:

1. Boarder to look like the form in the attached screenshot with dropshadow and rounded corners.
2. Align all labels and text boxes (reduce spacing).
3. Change hover to turn boarder of text box black. No background color.
4. Change buttons to match oval buttons used on site.
5. Remove WPmembers affiliate link.

This should be very easy for a CSS wiz which I am not.

Basically the form is 2 steps, i want to reduce that to 1 step. So all fields (name, phone, email, date and comment fields) should all be in 1 box, instead of 2. Hope this makes sense.

This request has 2 tasks:

1) Reconstruct the following form http://img258.imageshack.us/img258/455/thingy.jpg (including values, which are quite self-explanatory) using Niceforms. Documentation here: http://www.emblematiq.com/projects/niceforms/ (Download here: http://www.emblematiq.com/niceforms/v20/niceforms-v2.0.zip ). Fall in line with the colors used in the original form.

2) Make the form work with the Synxis Booking Engine (this is easier then it sounds as the link just needs to go out right and you’re done, documentation provided). i.e. The link will need to be something along the lines of synxis.com/booking.php?month=x,adults=x, etc.. etc.. *the documentation has been attached, so you’ll see how easy it is*.

For anyone with skills this is a one hour job as all the information and tools necessary has been provided to you (so I’ve already done half the work). If you understand the task, put a + sign at the end of your bid.

I am looking for someone to convert a PSD file into a css based form that can be either emailed when the submitt button is hit or the data collected in a file with out the use of a database if possible. I am looking for a programmer suggetsion on what method best fits my needs. I would like to be able to somehow collect all of the submitted data and populate in a text file. Looking for an easy way to combine multiple submission into one file for review.

The form is more of a customer survery that past customer will fill out. The form includes radials, drop downs, checkboxes, text area, required fields. I am only looking for a programmer, i have the design that i want done, just need someone to help with programming of the form. It needs to be in format (NOT INLINE CSS – so hard to edit) that a basic web designer will be able to edit easily and make minor adjustments to. When winning bidder is chosen, i will resend a finalized psd file. However there is a temp psd attached for basic understanding.
