My name is Marty and I am looking to hire you for a very easy website comment box clone and website upload that should not take you more than a couple of hours.

You do not have to design site. I have already created a Review Website with content, but I would like to have a template page where a customer can submit a review of their own. Here is an example.

I am looking for someone to clone the submit box from this page and format it for my site. The submited reviews should appear on each individual products review page that has already been created. A format example for the look of the reviews can be found here.

Once done, if you could upload the entire site and test everything to make sure the site is fully functioning that would great.
