Would like to reproduce this hearing test for my site:

I can change graphics but I need the functions. I think the example uses JQuery. Could use Flash if necessary.
Will only be accepting developers with good feedback on this one. Please be sure to examine the example thoroughly. It includes a speaker setup and test sounds at particular frequencies as well as a results section.
