We need a nice look online browser application created in php and mysql.

We like this idea (http://www.simplecustomer.com) but just need it with a few more features.

We want it the following sections

1) CLIENTS
2) SALES
3) PROJECTS
4) REPORTS

In clients we will enter our client details here with the option to delete/edit/add. Clients details would include (Company name, contact, address, telephone, mobile, fax, email, website, notes). In the clients section you can view all, or search for a name. if you click on a client it will show all appointments/notes/projects for that client.

In Sales section we want user to be able to enter appointments & notes per client. A calendar where they can enter their appointments and times. Then a notes section per client – where the user will enter details after a telephone call or appointment. An outcome box would be good also – if a sale is made it is checked – and this is then used for report stage.

In the projects section you should be able to add/edit/delete a project, a project can have one of three status – these being :

Research Writing Virtual As

I need someone to do about 15-20 hours of research and writing (correspondence on the behalf of me) where they will be setting up telephone appointments, and organizing the appointments.

The job will entail e-mailing and setting up appointments for interviews over the phone for a new software service. We will want to have documentation of every e-mail sent to different bloggers, writers and others. The e-mails must not be boiler plate e-mails, but rather customized to each recipient.

Work begins immediately and will last for at least 3 months.

We will pay on every e-mail sent and will start-off of by pre-approving a list of contacts that you find.

We are looking to pay around about $3 USD for every 10 e-mails sent. In general, there should be about 400 e-mails sent each week.

Medical Directory

I am planning to make a localized directory of medical practitioners. This site would allow users/viewers to contact the doctors, book appointments, look for a particular doctor within their area and so on. I also wanted to include a rating system for each doctor along with a comment option.

This site would have numerous functions as well as the directory and it might have to be built up in phases depending on how a programmer sees to be able to complete this. I am not whatsoever knowledgable in webdesign nor programming, but i have a “dream” and would like the help of someone or a team of individuals that can help to make this come true.

Another portion of this site would include a symptom checker, i would like it to be as close to the checker found on the webmd.com site under the “symptoms” link. it doesnt have to be as pretty, but moreso functional and user friendly. although i would prefer it to be as close to theirs as possible. this would in “diagnose” the user and refer them to the appropriate facility based on their location, doctor rating, etc.

It would be nice to have an “ask advice” option where the user can actually chat live or via email with a doctor.
each doctor would have an option of upgrading their listing to a more detailed one where they can add pictures, information about their specialties, map of location, qualifications, etc.

Also i would like another section of this site to be dedicated to medicines, vitamins and medical supplies. although one cant just log onto a website and buy prescriptions drugs. they would only have access to vitamins and certain med supplies. although doctors might (depending on their rights) can buy medications and other medical supplies that arent offered to the users.

This would also require a 4-tier login system whereby there will be:

ADMIN – backend users such as website developers, myself, etc

DOCTOR – purchase power, access to exclusive doctor-to-doctor forum, confirming appointments, posting articles/editorials, creating/updating personal profile, etc

USER- person who registered can rate doctors, comment, book appointments, regulated purchase power, chat with doctor, etc.

VIEWER/BROWSER – can just browse the site, use the directory, view ratings and comments, use the symptom checker, basically everything wiithout being able to actually modify the site at all

Also a news feed would be required of this site. latest news in medical discoveries, etc. also with comments, ratings, and option to share with social networking sites such as myspace, twitter, facebook, etc. Users should also be able to search for particular articles based on doctors that wrote them, symptoms, and basically by any keywords they plug in

And finally a portion dedicated to “lifestyles” whereby one can see information about different fitness programs, diets, nutritional information (“foodometer” on webMD website, but this is only a new addition to theirs and looks to be a HUGE collection of information and might not be able to be included it on our site for the first of couple phases, once again I leave that all up to whats possible by the developers). There coule be a household remedies portion there as well, where people can see various remedies suggested by people. this might also have to be monitored by a doctor or a group of doctors before posting, just to make sure someone doesnt post something that can truely be harmful and i dont want to even spell out worst case scenarios!

Well, this is my plan and am waiting to hear from someone as to what this can cost, in phases and/or as a whole. Thank you!

Joomla Appointment Component

Hi

I have a tattoo studio that is in need of online appointment setting. I will try to explain what I am after to the best of my ability. The registered user will log in to the main site, after which they will have the ability to set an appointment. The user would start by selecting what type of appointment they want, tattoo, body modification, piercing (admin would need to be able to create several different types or appointments). Once the type of appointment is selected a list of artist that can do that type of work would be displayed. The user would pick their artist and then see a calendar displaying the current month and show what days that artist is available. The user would then select a day and see what times the artist has available on that day. The user could then select a time slot. After the time is selected we need to display a conformation page, and require a deposit if the admin has requested so. Once the user has confirmed the appointment date and time and made the deposit if so required then an email will be sent to both the user and the artist showing the date and time along with any deposit made.

Backend:

The admin needs the ability to have full control over all aspects. Only the admin has the ability to define the appointment types, deposit requirements (as in, if a user is not required to put down a deposit and amounts of deposits), which artist can do what type of work (tattoo, body modifications, piercings, etc…) and who the artist are. The artists need to be able to, upon log-in to the main site, see their calendar in a month, week, or day view displaying all appointments that have been confirmed by the artist. They also need to be able to see any request that have been sent over from the website and have the ability to either accept them or decline them (might have an issue with deposits here). They need the ability to use this calendar as their one and only calendar, so they will need to be able to add walk in appointments also. The ability to print the calendar in month, week, day view would be nice but not required. The front desk assistant would need access to schedule appointment for the artist also, not sure exactly how this would work.

I think I’ve covered it all, but if you have any ideas, they’re welcome.

Must be delivered as an installable Joomla! 1.5 component with all source code – I’d like to be able to edit it if necessary, so the code needs to be commented.

Joomla Appointment Component

Hi

I need a Flash component that will allow my clients to book appointments with me. The component needs to have…

Front End:

1. Registered users of the website have automatic access when logged in to the parent site. Non-registered users can create an account that does not require them to be members of the parent site, but offers to register them as a member using a check box for confirmation.
2. Once logged in, the client is taken to a month view calendar page and selects the day/time they wish to book an appointment. Days with available slots are highlighted.
3. They are taken to the booking page for the day they selected.
4. The customer sees the available appointments. Appointments already booked show text “Booked” (Names of people booked are only available in back end).
5. Client selects the type & length of appointment from a check list (type & duration options defined by admin).
6. Client clicks on the slot they wish to book, notified of total cost for service. (admin able to set specific charge for different types of appointments.
6a. Non-member customers have to fill out personal info to make booking, members if logged in, click thru.
8. Must be able to require captcha or not, as defined by admin.
9. Client (member and non-member) is taken to a page to pay form mandatory sitting fee (This feature should be able to be turned on and off, and amount able to be set/adjusted by admin). Admin should be able to insert text notice of terms of service here. Upon confirm payment, client should be taken to PayPal page, (not requiring an account to pay by PayPal) “Confirmed Booking” should will be handled by PayPal. Upon completed payment, client should be returned to page containing booking details.
10. Client receives an email confirming the booking ( – email text can be defined by admin, and must pick up fields from the database (eg.[NAME])
11. Client can cancel booking if necessary, with notice that PayPal Payment is non-refundable, unless canceled at least 7 days in advance of scheduled appointment.
12. Client is sent a personalized email reminder 24 hours before their booking – email text can be defined by admin. Must pick up fields from the database (eg.[NAME])
13. Client history to be available to client. (including total cost of services, refund issued, type of appointment, date)

Back End: (admin options)

1. Registered users have automatic login, unregistered can book, but and automatically added to site registered users.
2. Admin has ability to choose dates and times to be available for bookings.
3. Admin as ability to set up time slots for each day – multiples of 60 minutes.
4a. Admin has ability to set and change cost for type of appointment.
4b. Admin has ability to set and change cost for mandatory prepaid sitting fee.
4. Admin has ability to repeat time slots for a specific day (eg. same slots every Tuesday).
5. Admin has ability to set slot types.
6 Ability to send notice of refund issued and field to check refund issued, and amount.
7. Ability to make custom fields as required.
8. Overview screen for each day with tooltip or clickable link with each user’s telephone numbers.
9. ability to produce reports by date, containing customer name, address, phone, slot type, number slots/time used, and, sitting fee paid, refund issued and total cost of service.

Must be delivered as an installable Joomla! 1.5 component with all source code – I’d like to be able to edit it if necessary, so the code needs info tags included.

Must be fully functional. Must be fully configurable for color/style, width from back end control panel.

Would also like a quick appointment module with the following options available: Appointment Type Field and Cost field and sitting fee field, able to be configues by admin, with checkboxes for clients, and a clickable date able to be clicked by client, and a confirm button that brings customer directly to PayPal Payment Page to pay the mandatory sitting fee.

Appt Scheduler By Zip Code

Our small business involves visiting patients in their homes. We need a scheduler that will organize the patients homes based on zip codes, specifically the last 4 digits of the zip code, since any one zip code can be quite large and we need to group patients who are in close proximity to one another. This could involve grouping patients together who are on the border of one zip code, but can also be in close proximity to another zip code. That is why the last four digits should be the best differentiator. We would like to have a simple tool that allows the scheduler to schedule appointments using the following sequence: The software needs to enable scheduling appointments taking the customer’s location into account. 1)Client receives a call from her customer requesting an appointment 2)Client asks customer for the home address AND zip code of the home where she will have to go & service needed 3)Client enters address AND zip code in screen to determine the customer’s complete zip code with unique 4 digit identifier. 3a) Complete zip code with Unique 4 digit identifier is displayed on screen based on information entered. 4)Computer displays all appointments for today and beyond that are in the same or within close proximity of the zip code with unique 4 digit identifier. Appointments are arranged in date and then time sequence 5)Client agrees with the customer on an acceptable day and time to schedule the new appointment 6)Client adds the new appointment information into the database Design Features: Zones – Each zip code with it’s unique last four digits has a unique identifier. Website Polishing

Hello,

We are looking for some modifications to an existing website.
The website provides a service mostly to relaters where they can login and book appointments and download (after purchasing) floor plans and measurements of their listing.

We would like the following changes added/made:

A Page for clients to view floor plans and measurements via a link
They can put on their website, accessed only after they have played.

Mysql calendar database to sync with exchange server and in turn to Outlook, may need some server side work.

Some search engine optimization done.

Admin side to incorporate the registering and booking for clients.
An email would be sent to the newly registered with a login and password so they can login and then change the password and download their files. As it is now clients can register and book appointments but the admin is unable to do this for them.
The business is based on a phone call then we book the appointments so the transition to the website isn’t working as good as it should. With the admin able to start the booking process this will change.

Website layout isn’t working well with IE8 of Win7.

Sql Database For Appointments

5 offices connected to server by VPN need a simple MS SQL database to hold appointment data. The form is about 3 screen pages long and has 100-150 fields. Indexing will be needed for searching on approximately 10 fields.

On screen, the form needs to appear similar to the paper form they have been using. They will need to be able to print these as well and the output should look like the paper form.

I’d like to use a free flavor of MS SQL (SQL Express??)

I’d like to start immediately and be finished within one month.
