Lead Submission Form

I need someone to simply do the coding for just the form portion of a simple lead website. The information will then post in a way that it integrates with an account we have with leads360, our lead management software.

Several things are essential:

First, the form should route the data to the correct lead database based on a unique campaign ID. I need to make sure that it changes the post URI/URL so that when they select where they found our company from a pull down list, it sends the lead to the appropriate campaign.

Second, it needs to dynamically create a unique, random, non repeating lead ref id number for each lead submitted. This will be submitted as one of the fields, although not visible to the lead.

Lastly, there are two types of lead submissions – those that already have an anonymous quote that they are following up on, and they will submit a quote id number and the form will essentially just request and then submit their contact information to the existing anonymous quote in the database, and those that are submitting a fully fresh quote request.

I want it so that it does this:

2 or 3 Pages, as applicable:

Page 1 “where did you find us”

Page 2, based on response:

Input just contact info and anonymous quote id number – then submit

or

input full new lead form broken down into two pages

page 2, basic info about what they are requesting and

page 3 basic contact info – then submit.

After submission all forms should reroute to a thank you page.

I will build the basic site, including the inputs and drop down menus, etc- I ONLY need you to correctly configure the forms so that they send the data properly.

The lead management company said the way it would distinguish between whether or not it was adding to a lead that was in the system already has to do with “post” or “get.”

It is also capable of handling “xml” posts if that is easier.
