Add Aweber Code To WordPress

My site is built on a custom WordPress theme. I currently have an email opt-in box on the homepage, and in the sidebar. The email service I use is Aweber.
The code has become screwed up, so I need someone to integrate new Aweber code in the homepage and sidebar.
This should be a five-minute job for someone who knows WP and Aweber.
