I have a quiz containing 5 questions each with 2 or 3 answers that the user can select via radio buttons.

Once the user has selected the answers they would then click on the “submit” button at the bottom of the page.

I would like to have the opt-in form come up by a lightbox (or something that performs the same function) once the they click the “submit” button.

All questions should be “required”.

The biggest requirement is that depending on how they answer the final question I would like to send them to 1 of 3 separate lists or opt-in forms.

So, the opt-in that pops up will depend on how the final question is answered.

I would be a great added touch to have a active graphic following the “submit” button and before the opt-in form appears to show that the system is “thinking”. (see attachment)

This would also have to be 650px wide.

The form is to be used on this website : http://zango.dk/TEMP/faq.html – so its important any tags etc dont overlap or overwrite the exiting styles.

The form should work perfectly in explorer, firefox and chrome browsers.

I need code written that would populate a search, probably Google, with search terms that are input by a user’s click on a separate list of words. For example, imagine a list including the terms “Spain, Germany, England, castle, sunny, rainy.” The user clicks Spain, castle, rainy, and Google searches for those terms.

I must be able to alter the code so that each of the words in the list inputs what I need it to input. For example, I will code the word Spain to search for ‘Espana’, castle to search for ‘castillo’, etc. so that when the user clicks on the English search terms, the foreign language search terms are actually input as the search.

Of course the user will have to be able to add as many or as few search terms to the list before choosing to search.

Search results should appear on my own website using Google’s API, and redirect to the destination site in a new window only after clicking through one of the results.

If the list of words could be made to look like a tag cloud, rather than just a list, so much the better.

OK, this is a pretty simple project for someone who has experience with CSS and WordPress. Basically, what you will do is add stuff to styles.css of the theme I’m using and then will insert the buttons using div tags via HTML mode of WordPress editor.

To make your job even easier, I created a pretty clear mockup of what needs to be done. Space distances A-F need to be defined in CSS. I might actually decided to do 4 buttons instead of 3 but if you know what you’re doing that shouldn’t make a difference to you.

The mockup is attached and the button codes will obviously be provided to the winning bidder.

SPECIAL REQUIREMENT: you must be available at least ONE hour on Skype on workdays between 3PM-3AM EST for the duration of the project. The 1 hour will be agreed upon convenient time for both of us and will be used to quickly resolve questions, issues etc, if the need arises. This requirement is a must, so include your preferred hour in your bid and your Skype nickname. Also, if you flake on the agreed hour even once, I will consider that a breach of your bid and there will be negative consequences for you.
