Modify Online Form And Add Admin Approval Function

Form is located at www.sc vifa cilit y.com/event
Form should send email to form owner and a confirmation to sender.
It should also populate a table that an admin can check a box to approve the request.
The admin area needs to be created and needs pw protected login and checkbox for approval. Once approved it should send email to an email address.
Also, on the form the style is inline. Need to have it changed so style comes from style sheet.
