I need a form built, that is basically like an employment application.

It will have 6-7 pages. Each page has approximately 15 questions (15 fields). Each field is basic form elements, such as drop down, check box, or text entry.

Basically, the user is presented with the “Step 1″ page of the form, which asks for basic information (name, address, phone, mobile, email, etc. etc).

This (and all other pages of the form) must have AJAX type validation (NOT server side or javascript popup), which would notify the user that the field(s) were entered wrong, and it should turn that form field red.

After the user has successfully enter information on the first page and he/she clicks Next, it will present him/her with “Step 2″ Form Page, which has a whole different set of questions/fields. The same validation shall apply to this and all pages. This page (and all others) should have the ability for the user to click back, or forward, and the session should keep all the previously entered information in the form fields, so the user can modify if needed. This functionality should apply for up to 6 form pages.

If the user has successfully entered all info, after the form is submitted, it should store this information in a MySQL Database, and it should forward to a “Thank You” page.

The “Thank You” page, DB and DB User, and any other info, should be made in a local config.php file, in case it needs to be changed.

Lastly, I would provide a PSD file of the form grahpic design layout (will be a simple layout), and would need this to be implemented into the actual form pages, using CSS (I would want the pages built via css, using minimal tables as possible)

I have broken down this project into 3 parts. It is not complicated but I like to make sure programmers understand.

Part #1: Fix one login bug.

My site has 4 oauth logins (twitter, y!, G and FB) that all work but the FB has a small bug and is not working. The ‘fb connect’ page is not coming up and it was working last week. This needs to be fixed.

Part 2: I have attempted to add this photo services oauth api on my site – http://bit.ly/e4y0OB but I am getting errors. I am using open source php code from http://bit.ly/jbWDvq . I need to pull the user endpoints so that their photos show on my site. So when I login it should pull my photos from service onto my site. Not storing images on my site just links that display as photos. I can show many site that are using this api and everyone says its a simple process so I must be missing a step.

(Ignore part #3 if you do not know the twitter api)

Part #3: Add to the Twitter login so when someone logins in the photos will automatically be pulled into their user page on my site. Oauth login scripts for me but one of them is not working right (small bug). Would also like these oauth login files organized better from previous project. I also need 2 more oauth added asap. You did a twitter oauth login for me and would like to automatically grab user photos from twitter accounts. I have a local event coming up where I need people to sign up to sit at a booth in 2-hour time slots. I will need an admin area where I can enter the event info, the dates and the times on each day that need to be filled. For example, I need to be able to go to the admin and select a day or multiple days (Example: Friday November 25, Saturday November 26, Sunday November 27), then select a time increment (1 hour, 2 hour, etc), then select a start time (10:00am) and end time (5:00pm). Note that the start times and end times may be different for different days so the system will need to give me the ability to customize start and end times for each day of the event. Then the system will auto generate a user interface page that someone can go to and sign up for that time slot. When they sign up for the time slot they will need to enter their name, email address and phone number. On the schedule itself, it will only show the name. There also needs to be functionality for multiple people to sign up for the same time slot.

On the page, we should also be able to print the schedule so as people sign up, a schedule can be printed with the most up to date information.

This application will need to be reused for other events in the future so all of the information (including the page title) needs to be modifiable through the admin.

However instead of streaming the video to a flash player on the users profile, the user shall get an mp4 link so they can embed it into there own player or viewers can view on a desktop player instead. So there will be no front end profiles as there are on livestream. This will only have a admin/member area where members can manage there channels from and can get there link from. Our current server is a godaddy virtual dedicated server and as such, it does not meet the PCI Compliance requirements. That is why we need to move our web site to a godaddy dedicated server (which we are purchasing). The server runs on a Linux Red Hat Fedora Core 7 Operating System. The new dedicated server will run Windows Server 2008 for the Operating System.

Our web site is an OSCommerce web site (osCommerce Online Merchant v2.2 RC1). We will need to install the following (using versions that are PCI Compliant – preferably the latest version of each unless there is a valid reason for not using the latest version):
MySQL
PHP
Apache (or we are open to your recommendation)

We will need you to ensure that everything continues functioning on our web site after the move.
The currently installed add-ons include, but are not limited to the following:
- UPSXML Negotiated Rates Shipping Module
- A Custom built Freight Estimator
- We currently use a First Data Global Gateway API that handles the Credit Card Transactions for the online store, but I understand that this will not work for Windows Server 2008 – we will need to install the Web Server API instead.
