I need a webform containing:
“Name” editbox. Required.
“Email” editbox. Required.
“Confirm Email” editbox (if different from 1st email editbox, an error is produced). Required.
“Address” textarea. Required.
“Country” drop-down list. Required.
“Background Colour” drop-down list (black, red, pink, cream). Required.
“Item Model” editbox. NOT Required.
“Select Design”: There is a choice of several images (a hundred or so) that can be selected. To save space on the page, these can either be shown in an iframe or as “pages” (1, 2, 3, etc) 10 images at a time, but the number of shown images should be able to be changed with a variable. User selects thumbnail image and it shows a larger image (just like a gallery would) and it’s title is automatically added to an editbox or label. Something simple to install – perhaps something like thumbnails are put in 1 directory and full size images with the same name are put in another directory. Required.
“Size” checkbox group. (Small, Large, Extra Large). Required.
“Texture” checkbox group. (Rough, Smooth, Doesn’t matter). NOT Required.
“Shape” drop-down list. (circle, square, rectangle). NOT Required.
“Age” radio group. (20-29, 30-39, 40-49, 50-59, 60+). Required.
“Are you a previous Buyer?” radio group. (Yes, No, Can’t remember). NOT Required.
“Sex” list. (Male, Female). Required.
“Where did you hear about us?” list. (Google, Ad, Friend, Other). NOT Required.
“Upload Image” button.
“Comments” textarea. NOT Required.
“I agree to the Terms and Conditions” checkbox (if not checked, an error is produced). Required.
“Send me email updates” checkbox. If ticked, their email address is added to MySQL database. A section in the Admin Section will be able to send emails to all on this list. The ability to remove their address should be avilable in user’s emails to avoid spam. A maximum number of emails should also be set (30 max per minute) to avoid overloading server. This to be a variable in the script. NOT Required.
“Join our Affiliate Program” checkbox (NOT Required) with link to open details page in new window (_blank). This will allow user to join our affiliate program. An auto-response email will automatically be sent containg unique HTML code with affiliate ID to place on his web site. General text for this can be set in Admin Section or a text file. Their name/email address is recorded in MySQL database. An email is also automatically sent to us with his details, ID, etc. A section in Admin Section to enable follow-up emails to be sent. Security to stop cheating. A simple script, perhaps something like http://www.twsc.biz/php_affiliate_script.php.
“Captcha Image” to stop spam. Required.
When user clicks submit button, they are taken to paypal to pay. Paypal is essential but any other payment processors (such as 2Checkout, etc) would also be useful. State on PMB if you can do any others in addition to paypal and this may give you a better chance that your bid is accepted. However, paypal is most important. A confirmation email is sent to user and also an email listing all their choices, including any uploaded file as an attachment, is sent to me. Also sent to me is the affiliate’s ID from which the user came from. Note that this is the affiliate that promoted us so we can pay their commission and not the current user/new affiliate. Items are also added to MySQL database. Text can be edited either in the Admin Section or taken from text files.

If possible, the html code of the form should be separate from the php code. I will want to edit the form later. Also the .sql file should be well commented.
Each section should be separated with, for example,” // ########## COMMAND START” and ” // ########## COMMAND END” etc and operations CLEARLY commented throughout for any changes I may wish to make later.
The script must have a consistent look throughout so header and footer files must be included.
