Rebuild Mysql + Adjust Custom Wp Plugin

Existing MySQL phone numbers data base has to be rearranged – screenshot attached; (one of the fields has too much info and has to be separated). After it is done the existing custom plugin has to be adjusted accordingly.
Here is what has to be done:
1) Rebuild existing database and have the data stored in separate fields/columns
