Need a site that works like the following:

Someone comes to the site and searches for an individual by name or location, and as they search the system uses an ajax auto suggest to show people in the system. optionally, people can enter in a special 5 digit number that was assigned to the indivindual at signup to locate them quickly.

When they enter the number or locate the individual, they are taken to a brief profile page for that person to confirm who it is and where they fill out a form to contact them.

This form sends the message to that person and anyone else THAT person has indicated it should go to (i.e. “CC:”, so the message can go to more than one person) The person being contacted may also opt to attach something (a document or picture) on an autoreply for the person submitting the message.

Messages go to a user account on the site, along with a message notifying them that someone has sent them a message. The person holding the account can login and respond to the person and a history of their conversation is stored (like email).

This is to be a fairly simple and uncluttered site.

Regarding the form that is filled out, it would be great if there were a number of different forms that could be predesigned with varying information gathering fields, and the user could select which form they want to present to the person who is contacting them.
