Logo Design – Make An Image Like The Attached File

We need a design like the attached. We need to change the name and the tagline, but keep the rest of the same quality. this design must be created newly, no image elements can be used from the sample design.
Delivery must be in a editable photoshop or other file format. (please specify what tool you will be using with your bid.)
