Html Coding Of Two Pages

Hello!
Using layered PSD file provided, create two HTML pages:
Page one will include 8 open-ended data capture fields and4 drop down fields.
Page two will include several dynamic fields.
Pages will need to be capability and present well in all versions of IE7, IE8 and IE9, as well as recent versions of Safari and Firefox. Samples of the presentation from each browser will need to be provided.
PDFs of the pages are attached as examples. Actual files and data definitions will be provided when the job is awarded.
