Facebook Allow App To Get The Email Address

I’ve been trying to learn how to make a Facebook application where once the user clicks “ALLOW” it sends their email address to me/captured with my autoresponder.
I can do it all except get the email sent 
I’m wanting someone to either edit what I have done (so it works) or create a new one from scratch and show me how I can duplicate it on another app/website.
So I know you have read and understood what is required, please type COOPER in your response thanks so much.
