Editable .pdf Form (or Word / Similar Format If Easier)

Hi,
I have a simple 2 page questionnaire in .pdf format but I need it to be editable by members. (Please see the attachment).
I do not want them to be able to change/edit/delete the questions, but I would like them to be able to type in their answers and then save it and email it back to me.
It is mixed between some tick box answers, and text answers – please view the attachment to understand.
If it is easier to make the document in another format, that is fine. As long as it can be opened by everyone on a standard computer without the need for special software.
