Virtuemart User Account Approval

I need someone to follow this thread to enable admin approval for virtuemart registrations.

The register link needs to go directly to the VM registration using this mod

The Virtuemart registration fields need to be edited/added to so they are
1. Full Name of Applicant.
2. Full name of Organization.
3. Postal Address.
4. Shipping Address.
5. Physical Address.
6. Names & addresses of Directors.
7. Telephone numbers.
8. Cellphone numbers (Company)
9. Cellphone numbers (Personal)
10. Fax Number/s.
11. Company e-mail address.
12. Applicants Personal e-mail address.
13. Website URL:
14. Certificate of incorporation number and copy.
15. PIN Number & Copy.
16. VAT number & Copy.
17. CCK license number and copy.
18. Organization business & activities.
19. Other dealerships in hand.
20. Estimated ICOM sales per month.
