Create Custom Optin Box And Lets Connect For WordPress

I am looking for a very specific project to be done. It is very simple but very detailed.

I need a customize opt-in box similar to the one in the attached file called “Mori Langshaw Sr-v2.jpg. The customize opt-in box should read “Download Mori’s 9 Realities For Home Based Business Owners – Embrace This First Or You Will Fail!” I use aweber.com for my autoresponder service, which should be linked to the opt in box. The size of the opt-in box should be around 250×250 (or the size of the ‘find us on facebook’ box on my site —> www.thenetworkersspeaker.com

I, also, need a customized Social Media box EXACTLY like the one in the attached file ‘Mori Langshaw Sr-v2.jpg”. The only difference is that it should be the logos with no words. IE. There should be a facebook logo then beside that a twitter logo. Under the facebook logo, should be a youtube logo and beside the youtube logo, should be a LinkedIn logo. It should look EXACTLY like the “Lets Connect” section in the file ‘Mori Langshaw Sr-v2.jpg’.
