Simple Asp.net Page 2

Basically i need an asp.net 2 page which save the record to a database ( oracle 10g and sql server 2005)
The page is a simple form which saves a record to the database with facility to upload multiple files
against each record and the path and name of uploaded files saves with the record and the files save on
a folder on server , the files should be saved with a unique name to avoid overwriting but the users should
see the original file name. This is a very simple requirement and its very urgent.
All the source code should be avaialable
please check the screen shot attached with all the requirements
