I need a chained select list using ajax, PHP and MySQL using a chained script.

The chained select is for reporting faults within a hobby sector. The user will select type, make, model and select from various other questions then post the form.

All selects will come from mysql database, then each selection should be stored in db for use at a later date.

When the user returns to same page he/she will be able to view selection previously made and able to edit selection + add further reports so a secure login, pass reminder function, captcha is required also. Again updating db if selection has been edited or further reports added.

On a seprate page we want to be able to view the reported faults, by type, make, model, and possible other paramters plus using a graph to show this data.

1. Interface but only if it can’t automatically detect the airport interface
2. Mac address to use (if left blank a randomly generated mac address should be input)

After the mac address has been changed the app should be able to reconnect to the previous internet connection automatically.
