Form Validated By Database W/ Paypal Payment

I need a web form that will collect data then validate the information with a database. Then when all data is validated by the database the form should then collect a PayPal payment. Once payment is collected the user is forwarded to a successful payment page.
I do not have a database established, but the fields for the form would be:
EBAY ITEM # (text field)
CASH SQUARE ID # (text field)
CASH SQUARE VALUE (text field)
CASH SQUARE GRADE: SILVER : GOLD : PLATIUM (drop down menu)
I would also need an admin area that would allow me to enter new information, edit info, and delete info from the database.
