Database With Admin Tool And Reports

PROJECT MUST BE COMPLETED BY SUNDAY 11PM EST.
Need a database setup with the fields I send over and a user login system. When the user logs in the report will be displayed from the data in the database.
There will be an Admin tool for adding users (username/password) and also for adding data to the database.
