Classified Website Programming

I need a programmer that can make a form where customers can submit the details of their ad. I want that information to be be able to be reviewed/monitored by me. If I approve it, I want the information that the user submitted via the form to be auotmatically put into a mysql/or other database. After the information is put into the database, I want a page to be automatically created based on a template. (similar to the way amazon.com does for its products).
The automated page should look similar to this with photos and everything:

Must have a portfolio and be fluent in english. Please send me your portfolio
