Php Form Customization/troubleshooting
When I first tested this form http://www.assistech.com/translators/free-quote-request.php , everything was working fine. Now, when I submit the data it gives me the following page error:

Permission is denied only on IE. On Firefox captcha is not validating. (I use the same captcha on another form and I don’t have this problem.)
Besides fixing this glitch, please make sure that the file(s) uploaded by the customer are attached to the email sent to me. Also, make sure that the Additional Services dropdown menu allows multiple selection and that the multiple selection is included in the email confirmation.
I also want to allow only these file extensions to be uploaded: .doc .docx .xls .htm .ppt .pdf .jpg .gif .png (No .exe please)!
The free-quote-mail.php file has a write function to write to quote.csv. There is no need for such function so it must be deleted.
I also use a Portuguese version of this form and I need to know what changes have been made so can fix it.
