Report Cover And 3d Picture

I have created a free PDF report to give away from my website. The report is entitled ‘Make today the last day you ever worry about your weight’. It is aimed at a female market and any images would need to be appealing to women of all ages.
I am looking for someone to create the following:
An attractive looking single page 2d cover that I can attach to the front of this report.
A 3d image showing a book with the same cover (tile on the front and the spine) which I can post on my website to entice people to download the report.
