Mobile App – Iphone/droid Db

I need an app driven with about 5 screens and that is datadriven from a database… we have sql server installed on a 2000 win server and we have a 2003 windows server.
Programmer will develop and will release all code, project files, and will also give a flowchart describing program.
All code will have notes in code to describe work done.
